It shows all of the steps on the back end, but on the front end I only get Login and End. Maybe you disabled something which allow customer to edit their profile. You can also take a look at the ACL in case you put some restriction on certain part of the site. About the shipping I will suggest to create a custom field with the option so the customer can decide. Custom fields are available with non free versions.
