You will have to create as many shipping methods as needed, in your case 6 should be enough. 1. Zone: EU - Maximum price: 100 - Fee: 10 2. Zone: EU - Minimum price: 100 - Fee: 0 3. Zone: Norway - Maximum price: 100 - Fee: 15 4. Zone: Norway - Minimum price: 100 - Fee: 0 5. Zone: Switzerland - Maximum price: 100 - Fee: 15 6. The payment method (a collect on delivery) will set the order status to this "quote request" and you will be able to manage it in the backend afterwards.
