You need to go in the menu Display->Custom fields and create a new custom field of the table "order" and the type "textarea" and enabled on the front end and you will see it on your checkout (Business edition only). I found you need to specify Custom field - type: item to do this. How can I limit this field to being displayed only for a specific product or product group?
