For the others:
You have to create custom field via the menu Display > Custom fields in the table "item" select the type "Ajax file".
This way, on the product page the customers will be able to upload their own photos to put on tee-shirts, mugs, etc.

The "ajax image" custom field does not support multiple images, you can upload just one image per custom field.
We will maybe improve that in the future, but it will break the way that the filenames are stored in the database.

Thanks for the report.
When an upload failed with the ajax uploader, the item you tried to upload show stay listed with a red background.
In the original design, the error message where displayed in this block but the actual implementation does not return a serialized error message to let the javascript display it.

I created a feature request for the ajax upload so we will improve that part.
We started to work on a "chucked" feature for the uploader, so when a file is bigger than your maximum upload size, the uploader will chuck it into several pieces, send these pieces one by one and let the server recreate the entire file.
