Will changes that I make to that file be replaced when I update hikashop?

Hi,

Yes, they will.
If you don't want to loose them, you can make a copy of the file to templates/YOUR_TEMPLATE/hikashoppayment/paypal_end.php
That way, that copy will be used instead of the core file and you won't loose your changes when you update your HikaShop.
