Option to add personalization to clothing product

Is there a way to add an option or characteristic that will allow a person to select it, then have a text entry box open so they can type a name or phrase into it ??

Here's the scenario. The customer sells embroidered T-Shirts with an association logo, they would like to be able to add a line or two so that a purchaser could include a line with their name, and possibly a second line with their title.

Is this a capability Hikashop has ??

Thanks ... I upgraded to Business so I can use this feature. Now, here's the kicker:

The customer wants to charge for adding this line, could you point me to the documentation on how to add this as a added-price option?

You could add an option so that the user would be charged more if he selects that he wants the embroidered text and then have your custom field being optional so that he can fill it when he selects the option.
