Customer image upload not displaying on checkout

I added a custom field that's an item-> Ajax Image. I can upload the image on the purchase/add-to-cart page but when I go to checkout, the area by the cart/product display shows the image as missing (question-mark box indicating the image can't be found). When I inspect the element, it says it's calling this:

Any idea as to what's happening?

That's a problem with the settings of the custom field in the database.
Edit your custom field via the menu Display>Custom fields, check that the display settings are what you want them to be and save the settings.
It should then work properly when you try with a new upload/add to cart.

The display settings for the image upload custom field? Because everything is 'yes' right now and I am not noticing any areas that seem like they would fix it if set to 'no'.

The key point here is to save the settings so that it properly set the data of the custom field in the database.
You can leave all the display options to yes for now.

It appears it may have primarily just been a cache issue. I cleared the cache and expired cache and it seems to be working now. Thanks.
