charging insurance on a shipment to the customer

Using the USPS postal coding system, where do I charge postal insurance on an order? Is there a place to have the customer choose to pay for insurance upon checkout? This would need to be a calculated amount based on the total value of the sale based on the postal tables from USPS. So this is a 2-part question of (1) calculating and charging the insurance and (2) giving the customer a choice of having the insurance. It also would be in both the Order and the Shipping Method categories.

There is actually no option to dynamically add an insurance price based on the USPS shipping method's price, but a solution can be to directly use the "Price" and "Percentage" options to adapt your USPS shipping prices.
