ajax upload fields showing on checkout

On the checkout the AJAX upload field is showing twice on the checkoutpage under "additional information" block:

jouwnaambord.nl/aluminium-naambord-zakelijk

(add some photo to go to the next step and see)

From what I can see, you have configured two custom fields of the table "order" and that's why you see two of them.
Please check the Display>custom fields menu.
