If you're asking how to add menu items using PHP code in your extension, then what I can say is this:
Joomla doesn't have any API for that.
So you can directly insert the data of your menu items in the joomla menus table.
That's what we do on our end. For example, in the function addMenus of the file install.hikashop.php that you can find at the root of the install package of HikaShop.
