Customize registration form

I want to customize the registration form page (via menu) to add custom fields.

I want the account to be pending approval

This is for people to sign up for wholesale accounts.

Any ideas would be greatly appreciated.

You can create custom fields of the table "user" or "address" via the menu Display>Custom fields and they will appear on the registration form.
If you want new accounts to be pending approval, you can set the option "New User Account Activation" to "admin" in the options screen of the Joomla user manager.

Is there a way to give them a login form? Or make the default joomla login redirect to the hikashop reg page?

You can turn on the "Redirect Joomla registration to HikaShop plugin" in the Joomla plugins manager and use the standard Joomla login module.
