Customer Account - Add New Address was created by gpraceman

-- HikaShop version -- : 2.4.0
-- Joomla version -- : 3.4.1
-- PHP version -- : 5.4.39
-- Browser(s) name and version -- : Firefox 37.0.2

When adding a new address, the modal popup opens at the bottom left of the screen and partially off of the page. The margin-left is set at a negative value. You cannot see but the left edge of the form fields, so the form is unusable.

Hi,

Please use another popup mode in your HikaShop configuration (section display) because your template seems to not be compatible with the Joomla bootstrap popups.
