how do you make a text box where customers can write instructions to the order?

Hi there,

Custom fields should do the job.
Just go in Display>Custom fields and create a new custom field on the table item.
Choose the type text or textarea and enable the front end display.

There is no need to code anything. Eliot told you that you just need to configure a custom order field via the menu System>Custom fields for that.
