Breadcrumbs - Simplified not displaying all

With Breadcrumbs set to simplified the products parent category is not displayed in the breadcrumbs, however, when set to simplified: No, the parent category is displayed...although the url becomes rather horrible.

What have I missed in order to get the products parent category to display in the breadcrumbs? In the example given, Gift Vouchers should display as the category.
