Custom Product Item display issue

I have created a custom field via Display -> Custom Fields -> Item. This is a text box.

When the shopper purchases a product we want them to select a variant to charge for the customization and fill in the text box for the text to be used in that customization.

The item displays, and shoppers have been filling in information, but we are not getting that information displayed on either the front-end order display, order email acknowledgements, invoices, etc., nor in the back-end in editing and processing the orders.

I've attached screen shots of the configuration.

So where to go to troubleshoot this further?
