With that plugin you could have a free product added automatically to the cart when one or more products from a specific category would be in the cart.
However, you can't have the customer choose which product.

Otherwise, you could do it in two steps:
- have a mass action to add the user to a specific user group if the purchased product has a specific value (for example, you could have a custom product field to set if the product can lead to the gift or not).
- configure the products of that category so that the price is only for the other user groups so that people in that user group won't see the prices and thus the products will appear as free (or have a 100% coupon for that user group for 1 product of that gift category).
- configure a mass action to remove the user from the user group after the second free purchase (for example, if the coupon was used, or if the total was 0).
The drawback is that it will require two separate orders to handle the gift but that will allow the customer to pick the gift product he wants.
