Hide name and email fields of product contact form and other things

Hello,

We are planning to conduct a survey of customers who have purchased a certain product. We see that there is the possibility of making a form for each product through the product contact form.

We use hikashop for the sale of courses, the form serves among other things to evaluate the teacher. So we want it to be anonymous.

1.- Is there any possibility that the required fields that request the email and name are not shown? Can a default or random value be set by default?
But that in no way are seen in the form.

The questions to be asked in the form are evaluations with different scores ...

2.- Is it possible in some way, later, to obtain the statistics of the evaluations obtained? (scores from 1 to 5).

3.- We have multiple custom fields in each product, would it be possible to show them in the form? For example within a custom text field.

4.- Could an email be sent with the corresponding link to the form filtering it to users who have purchased a certain product?
