address popup dont works

Hello
when I want to place an order and need to add a new address and click on New address bottom , address popup dont work and i redirect to a page like attached picture.
how can i active address popup?

I think that you have a javascript error on your page, that's why the popup is not working properly.
A link to your website could help us to see from where the problem come.

You can too select another way to display the address fields, via the option in Configuration > Checkout > Shipping & Payment > Address selector.
