Option to add personalization to clothing product

Is there a way to add an option or characteristic that will allow a person to select it, then have a text entry box open so they can type a name or phrase into it ??

Here's the scenario. The customer sells embroidered T-Shirts with an association logo, they would like to be able to add a line or two so that a purchaser could include a line with their name, and possibly a second line with their title.

Is this a capability Hikashop has ??

Thanks ... I upgraded to Business so I can use this feature. Now, here's the kicker:

The customer wants to charge for adding this line, could you point me to the documentation on how to add this as a added-price option?

Is there such a way ??
