HikaShop custom fields (menu Display>Custom fields) allow you to ask questions to the user on the product, on the checkout or during the registration:

Yes and no. HikaShop has no standalone form system because it's not a form extension (you an find plenty of them for joomla).
What you can do though is to use the "user group after purchase" option of your products (Business edition only) to add the user to a specific user group after the purchase of the products and then only allow the display of the form (of a third party form extension) to that user group. Now the issue will be to link the form answers with the order of the user I guess, but I don't know the details of what you want to do, so I an't say if that would be a workable solution.
