Custom Field on product

Hi

Is there a way we can display a custom field on a product that asks a user a question.

On the link attached, I want to ask a user if they require graphic design services for that product.

I tried creating a custom field, but it only displays the output and not the actual dropdown.

If this is not how you set up something like that, is there a way to do it? We wish to ask users questions on a per product basis but not sure how to go about it.
