Configurable Product don't show correct options in Cart

Hi. I am trying to create a configurable product with 3 extra options. For example I have as product Phone and extra option Phone Memory and second extra option Screen Size. I made all step by step how it is shown hear www.hikashop.com/support/documentation/i...ct-form.html#options
The problem is that after I add a product in Cart it show all option as separate product and not as extra option for Phone.
How can I solve this one?

Ex. Corect version
1.Phone Motorola 30$
extra Memory 1GB 15$
Screen Size 21" 30$


Wrong version:
1.Phone Motorola 30$
2. extra Memory 1GB 15$
3. Screen Size 21" 30$
